Jay Chaudhari, PT, DPT
Physical TherapistJay Chaudhari went to University of Illinois at Chicago for undergraduate studies. He earned his doctor of physical therapy from Midwestern University. Jay has a true passion for working in an outpatient setting in which he can help individuals achieve their functional goals and outcomes at a personal level. He believes that physical therapy plays a crucial role in treating a diagnosis in a non-invasive way to get individuals back to their daily lives.
Jay loves spending time with his family and friends. He also loves to travel and is a big football fan, his favorite team being the Chicago Bears.

Maria Lewis, MA
Medical AssistantMaria Lewis is a Registered Medical Assistant with 11 years of experience working with patients. She believes that patient care is one of the most important parts of her job. She loves putting a smile on patients’ faces and giving them the best care possible. Maria is very happy to be a part of the Union Wellness Centers and looks forward to bringing her experience to her new role.
Maria is a mother of 4, and enjoys family time, outings with friends and family, camping, bowling, and barbecues.

Sandeep Kattar, MD
PhysicianSandeep Kattar is a board-certified family practice physician with over 8 years of experience in different settings of healthcare. As a family practitioner, he treats a wide variety of ailments as well as focusing on the importance of preventive care. He believes in the importance of compassionate primary care that encompasses physical, emotional, and mental well-being.
He grew up in Michigan, and he and his family have been in the Chicago area since 2004. He is an avid sports fan and enjoys vacationing and outdoor activities such as hiking, camping, and fishing.

Tina Mikulecky, RN, BSN, MSN
Tina Mikulecky brings 30 years of nursing experience to the Union Wellness Centers. She received her bachelor’s degree from Lewis University and her master’s degree from Elmhurst University. Tina is a board-certified medical/surgical nurse and clinical nurse leader. She uses her leadership education and experience to encourage a team where everyone is valued and appreciated. Her goal for her team is to have every patient encounter be a positive one.
Tina has lived in the Chicagoland area her whole life with her husband of 31 years, and has two grown sons and two sweet dogs. In her spare time, Tina likes to relax by crafting, reading, and spending time at the lake with her family.

Logan Trento, BSHCL, MHA
Operations SpecialistLogan Trento received his master’s and bachelor’s degrees in healthcare administration from Valparaiso University. During his undergraduate and graduate studies at Valpo, Logan held an on-campus job as a building manager. As a building manager, Logan oversaw the daily logistical, technical, and operational functionalities of the 202,000 sq. ft. student union center on campus that provided services to 5,000+ students, faculty, and customers. During graduate school, Logan was offered an internship with Porter Regional Hospital where he worked until he completed his degree.
Logan was born and raised in Mokena, IL and still resides there to this day. In his free time, Logan enjoys playing in softball leagues, hanging out with friends, and watching sports, especially football.
